Job summary

Chestnut Health Company is seeking a Physician Assistant to join our Physiatry team in Illinois. You will collaborate with therapists and nurses to build comprehensive rehab plans and conduct patient assessments to set attainable goals for functional recovery.

Qualifications

• Active PA license in the United States; Illinois license preferred
• Current DEA license preferred (required upon start)
• Current Controlled Substance license preferred (required upon start)
• NCCPA certification preferred; PA-C credential preferred
• Interest in Physiatry/Physical Medicine and Rehabilitation is desirable
• Orthopedic or Skilled Nursing experience is a plus

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement rehabilitation treatment plans
• Conduct patient assessments including histories and exams to determine rehabilitation goals
• Monitor progress and adjust plans to optimize functional outcomes and independence
• Participate in multidisciplinary meetings, conferences, and continuing education
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR
• Educate patients and families about rehabilitation process, outcomes, and post-discharge care
