Job summary

Chestnut Health in Illinois is seeking a compassionate Physician Assistant to join our physiatry team, delivering patient-centered care in Physical Medicine and Rehabilitation. You will work with therapists, nurses, and physicians to develop comprehensive rehab plans and monitor progress to optimize functional outcomes.

Ideal candidates are eligible to work in the United States, with an Illinois PA license preferred, and a commitment to multidisciplinary collaboration, education, and patient

Qualifications

• Eligible to work in the United States.
• Active Physician Assistant license in Illinois preferred (required upon start date).
• Current Controlled Substance License preferred (required upon start date).
• Current DEA License preferred (required upon start date).
• NCCPA certification preferred.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
