Job summary

Chestnut Health in Jacksonville, FL seeks a compassionate Physician Assistant to join our Physiatry team. You will collaborate with therapists and nurses to design comprehensive rehabilitation plans, conduct assessments, and monitor progress to restore function and independence.

Qualifications

• Eligible to work in the United States.
• Active Physician Assistant license; Illinois license preferred.
• Current Controlled Substance License preferred (start date).
• Current DEA License preferred (start date).
• NCCPA Certification preferred.
• Interest in Physiatry/PM&R; Orthopedic or SNF experience is a plus.

Responsibilities

• Collaborate with therapists, nurses and other professionals to plan care.
• Perform patient assessments, histories and examinations to set rehab goals.
• Monitor progress and adjust treatment plans to maximize independence.
• Participate in multidisciplinary meetings and ongoing education.
• Document evaluations, plans, progress and discharge data in EMR.
• Educate patients and families about rehab process and post-discharge care.
