Job summary

Chestnut Health Company in Illinois is seeking a Physician Assistant to join our Physiatry/Physical Medicine and Rehabilitation team. You will collaborate with therapists, nurses, and other professionals to develop and implement comprehensive rehabilitation plans that restore function and independence.

You should be actively licensed in the state, hold a current DEA and CS licenses, and NCCPA certification. Preference for Physiatry experience; orthopedic or skilled nursing experience is a plus.

Qualifications

• Actively licensed to practice in the state of employment.
• Current Controlled Substance License required on start date.
• Current DEA License required on start date.
• NCCPA certification required.
• Desire to work in Physiatry/Physical Medicine and Rehabilitation.
• Orthopedic experience is a plus.
• Physiatry experience is a plus.
• Skilled Nursing experience is a plus.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine rehabilitation goals.
• Monitor patient progress and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate documentation of evaluations, treatment plans, progress notes, and discharge summaries in EMR systems.
• Provide education and counseling to patients and families regarding the rehabilitation process and post-discharge care.
