Chestnut Health in Tennessee is seeking a Physician Assistant to join a physiatry team in Skilled Nursing Facilities. You will collaborate with therapists and nurses to craft comprehensive rehab plans, assess patients, monitor progress, and educate families about care and outcomes.

Qualificações

• PA-C certification required.
• Licensed to practice in the state employed.
• Current DEA license required upon start date.
• NCCPA certification.

Responsabilidades

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans.
• Conduct thorough patient assessments to determine rehabilitation goals and interventions.
• Monitor patient progress and adjust treatment plans to optimize functional outcomes.
• Participate in multidisciplinary meetings and educational activities.
• Document evaluations, treatment plans, progress notes, and discharge summaries in EMR.
• Educate patients and families about rehabilitation process and post-discharge care.
