Chestnut Health in Kentucky is seeking a Physician Assistant to join our physiatry team serving Skilled Nursing Facilities. You will collaborate with therapists, nurses, and physicians to create comprehensive rehab plans aimed at improving function and independence.

New graduates are encouraged to apply. You will provide patient education, participate in multidisciplinary rounds, and maintain EMR documentation in a patient-centered, compassionate environment.

Formación

• Licensed to practice in the state of employed.
• Current DEA license required upon start date.
• Current Controlled Substance license required upon start date.
• NCCPA certification.
• Interest in Physiatry/PM&R.

Responsabilidades

• Collaborate with therapists, nurses, and other professionals to develop and implement rehabilitation plans.
• Conduct patient assessments and determine rehabilitation goals.
• Monitor progress and adjust treatment plans to maximize independence.
• Participate in multidisciplinary meetings and continued education.
• Maintain accurate electronic medical records and documentation.
• Educate patients and families about the rehabilitation process and discharge planning.
