Greater Good Health is seeking a Physician Assistant to provide high-quality, patient-centered care for senior Medicare patients in Roseburg, OR. The role involves a mix of in-clinic and in-home visits, with telehealth support as needed, focusing on wellness visits and comprehensive chronic care management.

You will document histories and medications, perform tests, collaborate with the Primary Care team, and educate patients on preventive care while maintaining HIPAA compliance and timely

Competenze

• PA certification from ANCC or AANP.
• 2+ years of clinical PA experience in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care.
• Knowledge of chronic care management and medication options.
• Current DEA licensure / Prescriptive Authority post-hire.
• Active CPR/BLS Certification.
• Bilingual preferred – English and Spanish.

Mansioni

• Conduct annual wellness visits, mainly at the patient's residence, with telehealth support.
• Document medical histories, symptoms, and medications.
• Administer various tests and screenings.
• Collaborate with the Primary Care team to ensure holistic care.
• Ensure timely completion of patient records with EMR access.
• Provide patient education on preventive care and treatments.
