Job summary

SSM Health’s Waupun Clinic is expanding its outpatient Family Medicine team and seeks an APP (FNP or PA) to join five physicians and two APPs in a 100% outpatient setting. The role features daytime hours, flexibility for a 4- or 5-day week, and no nights, weekends, or holidays, with admin time built into the schedule.

The successful candidate will have PA/NP training, Wisconsin licensure eligibility, and family medicine certification for NPs, working in a collaborative, patient-centered

• NPs should be APRNs.
• Family certification required for NPs.
• Ability to obtain PA/NP licensure in Wisconsin.
• Collaborative team player.

Responsibilities

• Provide outpatient family medicine care in a 100% outpatient setting.
