Job summary

SSM Health Dean Medical Group – Waunakee seeks an experienced Family Medicine Advanced Practitioner for a full-time, outpatient role with your own patient panel. The position offers a flexible 0.8–1.0 FTE schedule and built-in admin time.

Qualifications

• 3+ years as an Advanced Practice Provider.
• Primary care experience is a plus; urgent care or internal medicine backgrounds considered.
• Wisconsin PA/NP licensure required; NPs need family certification and APRN.

• Provide outpatient family medicine care with your own patient panel.

SSM Health Dean Medical Group– Waunakee is actively recruiting an experienced, full-time Family Medicine Advanced Practitioner to join our team of 5 physicians and 1 APP. In this role, you will have the opportunity to hold your own patient panel. Previous clinical healthcare background in primary care is strongly preferred.

Your Qualifications:

• 3 years' + experience as an Advanced Practice Provider required
