Premise Health Inc. is seeking a Full Time Physician Assistant to provide care to client employees at our Health Center in Sunnyvale, CA. Schedule features a weekday pattern with no nights or weekends and a comprehensive benefits package.

You’ll conduct exams, perform procedures, develop care plans, educate patients, order tests, and coordinate with physicians to ensure continuous, high-quality care.

Qualifications

• Active state medical licensure in the applicable jurisdiction.
• Current NCCPA certification required.
• Maintains state licenses/certifications to practice.
• Federal DEA license (matching each state of practice as applicable).

Responsibilities

• Conducts physical exams and preventive health measures per guidelines and approved agreements.
• Performs procedures such as injections, suturing, wound care, and infection management.
• Develops patient care plans, records progress, and ensures continuity of care.
• Educates patients on treatment plans, health maintenance, growth and development.
• Orders and interprets diagnostic tests to assess clinical conditions.
• Collaborates with physicians and healthcare teams for comprehensive care.
• Prescribes or recommends treatments, including medications and therapies.
• Refers patients to specialists or physicians as needed.
• Maintains patient health records to ensure accurate and up-to-date documentation and records.

As a Full Time Physician Assistant, you'll provide care to client employees in our Health Center located in Sunnyvale, CA.

Schedule:

• Monday - Thursday 6:00a - 4:30p
• No Nights, No Weekends

What You’ll Do

What You’ll Bring

• Maintains state licenses/certifications to practice.
• Maintains Board of Pharmacy Prescribing License (if applicable).
• Current NCCPA national certification required.
• Degree from ARC-PA accredited program.
• Federal DEA license (matching each state of practice as applicable).
• State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration).
• Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services.

Premise provides its reasonable and genuinely expected range of compensation for this job of $133,200.00 - $166,500.00 annually. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors.
