Responsibilities

• Treat residents with disorders/deficits under PT scope after physician order and evaluation.
• Optimize the functionality and skills of patients.
• Complete documentation via Net Health.
• Collaborate with supervising PT on programs and supervision requirements.
• Attend care planning, staff, and family meetings.
• Assist in meeting OBRA Resident Rights guidelines for state/federal certification.
• Follow facility infection control policies.
• Complete other tasks assigned by manager/supervisor.

Key Responsibilities

• Responsible for the treatment of residents with noted disorders or deficits within Physical Therapy scope of practice upon receiving physician order and completion of evaluation

• Optimize the functionality and skills of patients

• Completes all necessary documentation via Net Health

• Collaborate with supervising PT developing programs appropriate to the needs of facility residents while fulfilling PTA/PT co-signature and supervision requirements

• Attend all required meetings including care planning, staff meetings, family meetings, etc.

• Assist the facility in meeting the standards necessary to obtain state and federal certification while following OBRA Resident Right Guidelines

• Follow facility infection control policies and procedures

• Complete all other tasks that are deemed appropriate for the role and assigned by the manager/supervisor

Qualifications

• Current or eligible Physical Therapist Assistant license in the state of practice
