Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in North Platte, NE. Full Time with a $5,000 relocation or sign-on bonus, plus Part Time or PRN options and flexible scheduling.

The role focuses on planning, organizing, and directing PT services to ensure high-quality resident care in compliance with standards, with duties including screenings, treatment planning, and resident/family education.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Familiarity with CMS expectations and NetHealth EMR is a plus.

Responsibilities

• Provide physical therapy services to residents under supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention or treatment.
• Assist in evaluations and treatment planning with the Physical Therapist.
• Develop target lists with short and long-term goals and treatment plans.
• Deliver individual and group treatments across mobility, strength, balance, and modalities.
• Assist in selecting techniques/media and adapt as needed.
