Job summary

Evolve Therapy Services in North Platte, NE is seeking a licensed Physical Therapy Assistant to support an in-house Skilled Nursing Facility. You will work under a Physical Therapist to deliver patient-centered care, conduct screenings, and help implement treatment plans.

This position offers flexible scheduling options (Full Time, Part Time, or PRN), with a $5,000 relocation or sign-on bonus for eligible candidates and opportunities for professional growth within the facility.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying treatment goals and plans.
• Conduct individual and group treatment as outlined in the treatment plan.
• Assist in selecting techniques/media and adapt as indicated by resident performance.
• Monitor residents’ response to intervention and modify as needed in consultation with the Physical Therapist.
• Establish and modify resident schedules with the team.
• Adhere to department and resident schedule; adjust as necessary for treatment regimens.
• Train residents, families, and staff in equipment use.
• Participate in resident, family, and staff education.
• Demonstrate sound judgment in planning and follow-up of therapy programs.
• Report to work on time and coordinate schedule for productivity.
• Assist with Quality Assurance programs as required.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.

Education

Associate's degree in physical therapy

Active Physical Therapy Assistant license in state(s) of practice
