Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility partner in North Platte, NE. This full-time role offers a $5,000 relocation or sign-on bonus and flexible scheduling with options for Part Time or PRN.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying goals and treatment plans.
• Conduct both individual and group treatment in line with the treatment plan (strength, mobility, gait, ROM, modalities).
• Assist in selecting techniques/media and adapt as indicated with consultation from the Physical Therapist.
• Monitor residents’ response to intervention and notify the Physical Therapist if modifications are needed.
• Help establish and modify resident schedules with the Physical Therapist and team.
• Adhere to department and resident schedules; adjust as needed for treatment.
• Train residents, families, and staff in equipment use and applications.
• Participate in resident, family, and staff education.
• Maintain sound judgment in planning and follow-up of therapy programs.
• Report to work on time and coordinate schedules for productivity.
• Assist with Quality Assurance activities as required.
• Complete required documentation in NetHealth EMR per CMS expectations.
