Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in North Platte, Nebraska. The role supports the supervising Physical Therapist in delivering resident-focused therapy services and implements treatment plans aligned with CMS expectations.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluation and treatment planning under the Physical Therapist guidance.
• Assist with short- and long-term treatment goals and plans for residents.
• Conduct individual and group therapy sessions including mobility, strength, balance, gait and ROM activities.
• Assist in selecting techniques/media and adjust activities with supervision.

Education
