Job summary

Evolve Therapy Services in East Moline, IL is hiring a Physical Therapy Assistant to support an in-house Skilled Nursing Facility. The role provides hands-on therapy under the supervision of a Physical Therapist and includes resident screenings, treatment planning, and patient education.

Open as Full Time, Part Time, or PRN, with opportunities for growth and ongoing professional development. Strong teamwork with therapists, nursing, and support staff is expected to ensure high-quality resident

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the PT's direction.
• Assist in developing target goals and treatment plans.
• Conduct individual and group treatment per the plan (strength, mobility, gait, ROM, modalities).
• Assist in selecting techniques/media and adapt as indicated.
• Monitor residents’ response and modify as needed in consultation with PT.
• Assist with resident schedules and coordinate with the team.
• Train residents, families and staff in equipment use.
• Participate in resident and staff education and QA activities.
