Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Antioch, TN. Eligible candidates may work full-time, part-time, or PRN in a supportive, patient-centered environment.

The role focuses on evaluating residents, developing treatment plans, and ensuring CMS-compliant documentation using NetHealth EMR. A BS in Physical Therapy and an active license are required.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice.
• Adhere to procedures regarding documentation and billing of physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention/treatment.
• Develop treatment plans with short and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents and the interdisciplinary team.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.
