Job summary

Sigma Health Rehab in Bath, NY is actively seeking a per diem Physical Therapy Assistant (PTA) to support residents in a Skilled Nursing Facility. You will work under supervision to carry out the plan of care and help residents regain daily function.

Qualifications

• Graduate of an accredited PTA Program.
• Current New York State PTA License.

Responsibilities

• Select therapy activities that follow the plan of care set out by the supervising occupational therapist.
• Assist residents in carrying out daily activities.
• Monitor residents' performance in therapy activities, provide encouragement and document on residents as needed.
• Teach residents how to use special equipment to assist with daily function.
• Collaborate with the PT to establish a rehabilitation plan.
• Maintains a cooperative relationship among healthcare team members & contributes to team effort.
