Sigma Health Rehab is actively seeking a Full-time Physical Therapist (PT) to join our skilled nursing facility team in Bath, New York. You will evaluate residents, develop rehab plans, and provide hands-on therapy to improve function.

Responsibilities

• Evaluate residents’ conditions & develop appropriate physical rehabilitation plans.
• Relieve resident’s pain using exercises, hands-on therapy & stretching.
• Help residents through their tasks and monitor their performance.
• Supervise the activities of physical therapy assistants.
• Maintain resident confidence & protect facility by keeping information confidential.
• Complies with federal, state, and local legal and professional requirements.
• Maintains a cooperative relationship among healthcare team members.
