Santa Cruz Post Acute is seeking a compassionate and motivated Physical Therapy Assistant (PTA) to join our rehabilitation team at our skilled nursing facility in Santa Cruz, CA. We offer full-time, part-time, and PRN opportunities with flexible scheduling, starting at $45–$50 per hour (DOE) plus benefits.

The PTA will deliver direct treatments, assist with therapeutic exercises, document progress, and collaborate with therapists and nursing staff to support resident-centered goals.

Qualifications

• Current Physical Therapy Assistant (PTA) license in California.
• Graduate of an accredited PTA program.
• Excellent communication, teamwork, and documentation skills.
• SNF experience preferred, but new graduates are encouraged to apply.

Responsibilities

• Deliver direct physical therapy treatments following plans of care.
• Assist residents with therapeutic exercises, gait training, mobility programs, and functional activities.
• Accurately document daily treatment sessions, resident progress, and responses to therapy.
• Collaborate with the Physical Therapist, therapy team, nursing staff, and interdisciplinary team members.
• Educate residents and caregivers on exercise programs, safety strategies, and mobility techniques.
• Maintain compliance with state and federal regulations and professional standards of practice.

We offer full-time, part-time, and PRN opportunities with flexible scheduling. Position is PRN starting at $45-50/hour (DOE) + Benefits.

Key Responsibilities

• Deliver direct physical therapy treatments following individualized plans of care developed by the Physical Therapist.
• Assist residents with therapeutic exercises, gait training, mobility programs, and functional activities.
• Accurately document daily treatment sessions, resident progress, and responses to therapy in compliance with facility and regulatory standards.
• Collaborate with the Physical Therapist, therapy team, nursing staff, and interdisciplinary team members to support resident-centered goals.
