Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Oklahoma City, OK. The role supports the Physical Therapist in delivering comprehensive therapy services to residents.

Responsibilities include conducting screenings, implementing treatment plans, and documenting outcomes in NetHealth EMR per CMS standards. Licensed PTA in state(s) of practice and ongoing professional development are required.

• Ability to communicate clearly with residents, families and team.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Identify indications for evaluations and assist with treatment plans.
• Assist in developing short- and long-term treatment goals.
• Conduct individual and group treatment per the plan (strength, mobility, balance, gait, ROM).
• Assist in selecting techniques/media and adapting as needed.
• Monitor resident response and modify as directed by PT.
• Help establish and adjust resident schedules with the PT and team.
• Maintain timely documentation per CMS expectations in NetHealth EMR.
• Train residents, families and staff on equipment use.
• Participate in resident, family and staff education.
