Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Webster, MA. The role supports the Physical Therapist and contributes to high-quality resident care.

Responsibilities include screenings, treatment planning, group and individual therapy, documentation in NetHealth EMR, and coordination with the care team. Requires an active PTA license and an Associate's degree.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.
• Maintain current licensure and certification through ongoing education.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist the Physical Therapist in evaluating residents and developing treatment plans.
• Assist in establishing and modifying individual resident schedules with the Physical Therapist and team.
• Perform both individual and group therapy as outlined in the treatment plan and modalities.
• Document compliant and medically necessary care in NetHealth EMR and communicate progress to the team.
