Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Hadley, MA. This full-time role (with a $3,000 sign-on bonus) offers both full-time and PRN options and requires an active PTA license and an accredited associate degree.

Responsibilities include providing therapy under supervision, conducting resident screenings, and assisting in treatment planning, scheduling, and documentation in NetHealth EMR systems.

Qualifications

• Must have an active Physical Therapy Assistant license in state(s) of practice.
• Associate's degree in physical therapy from an accredited program.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsabilités

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the direction of the Physical Therapist.
• Assist in establishing and modifying individual resident schedules with the Physical Therapist and other team members.
• Complete CMS-compliant documentation from CMS expectations in NetHealth EMR systems.
