Job summary

Evolve Therapy Services, LLC in Hadley, MA is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility. This full-time (or PRN) position supports the physical therapy team under the supervision of a Licensed Physical Therapist and includes documentation in NetHealth EMR.

You will assist with screenings, treatment planning, gait and mobility exercises, and resident education. A PTA license in the state and an associate degree are required; a $2,500 sign-on bonus is offered.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and treatment planning under supervision of PT.
• Assist in establishing resident schedules with the PT and team.
• Train residents, families and staff in use of equipment.
• Document compliant and medically necessary notes in NetHealth EMR.
