Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Hadley, MA. The role supports the Physical Therapist and contributes to resident care under supervision.

Full-time position with a $3,000 sign-on bonus; also available Part Time or PRN. Requires an Associate's degree in physical therapy and an active PTA license, with ongoing professional development and compliant documentation in NetHealth EMR.

Qualifications

• Minimum Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention and treatment.
• Assist in evaluations and in developing treatment plans with goals.
• Deliver individual and group treatments including strength, mobility, balance, gait, and ROM activities.
• Assist in selecting therapy techniques and adapt as indicated.
• Monitor resident responses and communicate with the Physical Therapist for modifications.
• Help establish resident schedules and coordinate with the team.
• Prepare compliant documentation in CMS-compliant EMR systems.
