Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Hadley, MA. This full-time position offers a $2,500 sign-on bonus or PRN option, and requires an Associate's degree and an active PTA license in MA.

You will assist the Physical Therapist in delivering therapy services, conducting screenings, and helping develop treatment plans while coordinating with the care team. Ongoing continuing education is encouraged to maintain licensure.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluations and development of target goals and treatment plans.
• Assist in establishing and modifying individual resident schedules with the PT.
• Deliver individual and group treatment including strength, mobility, gait, ROM, and modalities.
• Select techniques/media in consultation with the Physical Therapist.
• Monitor residents' response and modify treatment as indicated.
• Educate residents, families, and appropriate staff.
• Document CMS-compliant and medically necessary records in NetHealth EMR.
• Contribute to Quality Assurance programs as required.
