Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Hadley, MA. You will support the Physical Therapist, conduct resident screenings, and help implement treatment plans to promote mobility and independence.

The role requires an accredited PTA program degree, active licensure, and commitment to ongoing professional development, with opportunities for growth and a $3,000 sign-on bonus for eligible full-time applicants.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsabilités

• Provide physical therapy services to residents under the supervision of the PT.
• Conduct screenings to determine need for intervention.
• Assist in evaluations and treatment planning with the PT.
• Develop resident schedules with the PT and team.
• Document CMS-compliant, medically necessary treatment in NetHealth EMR.
