Job summary

Evolve Therapy Services in Hadley, MA is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility, full-time with a $2,500 sign-on bonus. You will work under the supervision of a Physical Therapist to deliver resident-centered care.

Responsibilities include screenings, treatment planning, gait and mobility training, and education for residents, families, and staff, with CMS-compliant documentation in NetHealth.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluation and treatment plan development with the PT.
• Lead independent and group treatments focused on mobility, strength, balance, and gait.
• Monitor resident responses and adjust interventions with PT input.
• Document CMS-compliant care in NetHealth EMR.
