Job summary

Freedom at Home is seeking a compassionate Physical Therapy Assistant (PTA) to provide skilled in-home physical therapy under the supervision of a licensed Physical Therapist. You will work with patients in the comfort of their own homes to improve mobility, strength, balance and daily function.

The PTA will develop and carry out individualized plans, monitor progress, and document care while collaborating with clinicians to ensure safe, high-quality care and patient independence.

Qualifications

• Graduate of an accredited PTA program.
• Current PTA license in the state of practice.
• Current CPR certification.
• Minimum of two years PTA experience preferred.
• Home Health experience is a plus but not required.

Responsibilities

• Provide skilled physical therapy services under the direction and supervision of a licensed Physical Therapist.
• Carry out individualized treatment plans to improve mobility, strength, balance, function and quality of life.
• Assist patients with therapeutic exercises, gait training, transfer training and use of assistive devices.
• Monitor patient progress and communicate changes to the supervising Physical Therapist.
• Educate patients and caregivers on home exercise programs, fall prevention and safe mobility techniques.
• Complete accurate, timely clinical documentation.
• Participate in case conferences, team meetings and quality improvement initiatives.
• Collaborate with physicians, nurses, therapists and other healthcare professionals to provide coordinated patient care.

As a PTA Home Health, you will provide skilled physical therapy services under the direction and supervision of a licensed Physical Therapist to patients in the comfort of their own homes.

Responsibilities:

• Provide skilled physical therapy services under the direction and supervision of a licensed Physical Therapist
• Carry out individualized treatment plans designed to improve patients' mobility, strength, balance, function, and overall quality of life
• Assist patients with therapeutic exercises, gait training, transfer training, balance activities, and use of assistive devices
• Monitor patient progress and communicate changes to the supervising Physical Therapist
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
• Complete accurate, timely clinical documentation
• Participate in case conferences, team meetings, and quality improvement initiatives
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to provide coordinated patient care
• Promote safe, compassionate, high-quality care that helps patients remain independent in their homes

Qualifications:

• Graduate of an accredited Physical Therapist Assistant (PTA) program
• Current Physical Therapist Assistant license in the state of practice
• Current CPR certification
• Minimum of two years of PTA experience preferred
• Home Health experience is a plus but not required
• Valid driver's license, reliable transportation, and auto insurance
