Job summary

Boost Home Healthcare is seeking a licensed Physical Therapist Assistant (PTA) to provide physical therapy services to patients in their homes under the supervision of a licensed Physical Therapist. The role focuses on mobility, strength, and independence within each patient’s home setting.

You will document progress, coordinate with the care team, and educate patients and caregivers on therapeutic exercises and mobility techniques.

Qualifications

• Graduate of an accredited Physical Therapist Assistant education program.
• Current Physical Therapist Assistant license or certification, as required by the state.
• Experience in home health or a related clinical setting preferred.

Responsibilities

• Provide physical therapy services as directed by the Physical Therapist and in accordance with the established plan of care.
• Assist patients with therapeutic exercises, mobility, functional activities, and other interventions within the PTA’s scope of practice.
• Help patients improve mobility, strength, function, and independence within their capabilities.
• Observe and document patients’ responses to treatment and communicate progress, concerns, or changes in condition to the supervising Physical Therapist.
• Collaborate with the Physical Therapist and other members of the home health care team to support coordinated patient care.
• Educate patients and caregivers on exercises, mobility techniques, and other activities as directed by the Physical Therapist.
• Complete required clinical documentation accurately and timely in accordance with agency policies and applicable requirements.
