Job summary

Freedom at Home is seeking a Physical Therapist for home health in Michigan. The PT will evaluate patients in their homes, develop individualized treatment plans, and provide skilled physical therapy services to improve mobility and function.

Responsibilities include educating patients and caregivers, completing documentation, and collaborating with a care team to deliver high-quality outcomes. A sign-on bonus is offered to eligible candidates.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Michigan Physical Therapist license.
• Current CPR certification.
• Minimum of 1 year of PT experience.
• Home Health experience is a plus but not required.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Complete OASIS and clinical documentation in a timely manner.
