Performance Home Health, LLC is seeking a compassionate Physical Therapist Assistant (PTA) to provide high-quality, in-home physical therapy services under the supervision of a licensed Physical Therapist. The role focuses on mobility, safety, and independence, in a flexible field-based setting with a comprehensive benefits package.

The ideal candidate will have a PTA program credential, active Texas PTA license, CPR, and reliable transportation.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current and unrestricted Physical Therapist Assistant license in the state of practice.
• Current CPR certification.
• Valid driver's license, current automobile insurance, and reliable transportation.
• Strong clinical judgment, communication, time-management, and organizational skills.
• Ability to work independently and effectively manage responsibilities in a field-based environment.

Responsibilities

• Provide PTA services under the supervision of a licensed Physical Therapist.
• Implement individualized treatment plans to improve strength, mobility, balance, and independence.
• Monitor patient progress and report changes to the supervising PT.
• Educate patients and caregivers on exercises, home programs, fall prevention, mobility, and safety.
• Complete accurate clinical documentation in the EMR.
• Maintain compliance with organizational and regulatory standards.
• Collaborate with the care team to support coordinated patient care.
• Promote patient safety and infection control in home visits.
• Travel to patients’ homes to provide field-based therapy.

Position Summary

Performance Home Health is seeking a compassionate, skilled, and motivated Physical Therapist Assistant (PTA) to join our growing home health team. This field-based position provides high-quality physical therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist. The ideal candidate is patient-focused, organized, and committed to helping individuals improve their mobility, strength, safety, functional independence, and overall quality of life. • Provide Patient Care: Deliver physical therapy treatments and interventions in accordance with the established plan of care and under the supervision of the Physical Therapist.
• Implement Treatment Plans: Carry out individualized treatment plans focused on improving strength, mobility, balance, endurance, safety, and functional independence.
• Monitor Patient Progress: Evaluate patient response to treatment and promptly communicate changes in condition, progress, or concerns to the supervising Physical Therapist.
• Educate Patients & Caregivers: Educate patients and caregivers on therapeutic exercises, home programs, fall prevention, mobility techniques, and safety practices.
• Complete Clinical Documentation: Accurately and timely document patient visits, interventions, progress, and outcomes within the electronic medical record (EMR).
• Maintain Compliance: Ensure all documentation and clinical activities meet organizational, regulatory, and professional standards.
• Collaborate Across the Care Team: Partner with Physical Therapists and other interdisciplinary team members to support coordinated, patient-centered care.
• Promote Patient Safety: Follow infection control protocols, patient safety procedures, and established standards of professional practice.
• Provide Field-Based Care: Travel through assigned service areas to provide therapy services in patients' homes.

Required Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current and unrestricted Physical Therapist Assistant license in the state of practice.
• Current CPR certification.

• Strong clinical judgment, communication, time-management, and organizational skills.

Preferred Qualifications

• Minimum of 3 years of home health experience.
• Experience utilizing electronic medical record (EMR) systems.
• Demonstrated ability to efficiently manage a flexible patient schedule.
• Strong knowledge of fall prevention, patient safety, therapeutic exercise, and functional mobility interventions within the home setting.

Annual compensation: $90,000–$100,000

• Mileage reimbursement at $0.725 per mile
