Evolve Therapy Services in Central City, NE is seeking a compassionate Physical Therapy Assistant to support an in-house Skilled Nursing Facility. You will work under the supervision of a Physical Therapist to provide patient-centered care.

Responsibilities include screenings, developing treatment plans, facilitating individual and group therapy, documenting per CMS requirements in NetHealth EMR, and coordinating with residents, families and staff.

Qualifikationen

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and licensure maintenance.

Aufgaben

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist the Physical Therapist in identifying goals and treatment plans.
• Contribute to individual and group treatment sessions as outlined in the plan.
• Monitor residents’ response to intervention and adjust as needed.
• Assist in scheduling in collaboration with the Physical Therapist and team members.
• Maintain timely, accurate documentation per CMS requirements in NetHealth EMR.
• Participate in Quality Assurance activities.
