Job summary

Evolve Therapy Services in Bradford, PA is hiring a compassionate Physical Therapy Assistant to support an in-house Skilled Nursing Facility partner. You will help implement treatment plans, conduct screenings, and work under the supervision of a Physical Therapist to improve residents’ mobility and quality of life.

The role requires an Associate’s degree in physical therapy, an active PTA license, and commitment to ongoing professional development.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Must have an active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services under the direct supervision of the Physical Therapist.
• Conduct resident screenings to determine need for intervention.
• Assist in evaluation and in developing treatment plans with short- and long-term goals.
• Deliver individual and group treatment focusing on mobility, balance, gait, and ROM.
• Train residents, families, and staff in equipment use and home exercise programs.
• Document care accurately in the NetHealth EMR and assist with CMS compliance.
