Job summary

Evolve Therapy Services is seeking a Licensed Physical Therapy Assistant to support an in-house Skilled Nursing Facility in Fostoria, OH. You will work under the supervision of a Physical Therapist to deliver patient-centered therapy and complete CMS-compliant documentation in NetHealth EMR.

The ideal candidate holds an Associate's degree in Physical Therapy, maintains an active PTA license, and demonstrates compassion, teamwork, and strong communication to enhance resident care and outcomes.

Qualifications

• Associate's degree in Physical Therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluation and treatment planning with the Physical Therapist.
• Develop and modify resident schedules for treatment with the PT and team.
