Job summary

Evolve Therapy Services seeks a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Dayton, OH. You will work under a Physical Therapist to provide therapy services and support residents’ mobility and functional goals.

Qualifications

• Associates Degree in Physical Therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in developing target goals and treatment plans with the Physical Therapist.
• Deliver individual and group therapy sessions per the treatment plan.
• Assist in selecting techniques and adapting activities per resident performance.
• Monitor residents’ response and consult with the PT for modifications.
• Help schedule residents’ therapy sessions and coordinate with the team.
• Document compliant, medically necessary CMS-related records.
