Job summary

Evolve Therapy Services, LLC seeks a Physical Therapy Assistant to support an in-house Skilled Nursing Facility in Warren, OH. You will provide PT services under supervision and help implement treatment plans for residents.

The role includes screenings, goal setting, group and individual therapies, and documentation in NetHealth EMR. Licensure and ongoing CE are required to maintain practice eligibility.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in identifying target goals and treatment plans with the Physical Therapist.
• Document CMS-compliant records in NetHealth EMR systems.
