Connect Pediatrics is seeking a Pediatric Physical Therapist Assistant to join our in-home care team in the Little Elm/Aubrey/Frisco area of Texas. You will implement pediatric treatment plans, provide in-home therapy, and communicate progress to families while collaborating with a multidisciplinary team to deliver comprehensive pediatric home health services.

Formación

• MUST hold a valid Physical Therapist Assistant license in Texas.
• Experience with pediatric therapy is preferred.
• Excellent communication with families and care team.

Responsabilidades

• Implement treatment plans per physician orders for pediatric patients.
• Provide in-home therapy services across a range of diagnoses.
• Engage with families and provide updates on goals and progress.
• Collaborate with a multidisciplinary team to develop holistic plans.
• Maintain thorough documentation of care provided and changes.
• Participate in regular meetings with supervisors to discuss patient progress.

Responsibilities:

• Implement relevant treatment plans aligned with physician's orders.
• Provide in-home therapy services for pediatric patients with a wide range of diagnoses.
• Engage with patients' families, providing them with regular communication and updates on goals for their child.
• Collaborate with a multidisciplinary team, including other therapy disciplines, physicians, and nurses, to develop holistic treatment plans that address the diverse needs of each patient
• Maintain comprehensive documentation that reflects the care provided and communicates changes in treatment plans to the entire healthcare team.
• Actively participate in one-on-one meetings with PT Supervisor/team leads to discuss patient progress, share insights, and refine treatment strategies collaboratively.

Requirements

MUST have:

• A valid, active Physical Therapist Assistant license in the state of Texas
• Current BLS/CPR License
• Reliable transportation, a valid driver's license, and automotive insurance
• Pediatric experience is preferred but not required
