One Home Health LLC is seeking a motivated Physical Therapist Assistant (PTA) to provide in-home therapy for patients in the Northside Suburbs of Chicago, IL. The position is part-time and supports an interdisciplinary team to deliver individualized care in patients' homes.

The PTA will supervise prescribed treatments, monitor progress, and maintain compliant records while traveling across assigned areas. APTA-recognized PTA program completion and IL license are required, with strong

• Provide prescribed PTA treatments and education under the direction of the supervising Physical Therapist and according to the plan of care.
• Monitor patient progress and communicate changes in condition, treatment response, or scheduling needs to the appropriate team members.

Service Area: Northside Suburbs of Chicago, Illinois, including Lake Forest, Glenview, Buffalo Grove, Mundelein, and nearby communities.

One Home Health is currently looking for a motivated and caring Physical Therapist Assistant (PTA) to support our home health patients on a part-time basis. In this position, you'll work closely with a dedicated interdisciplinary team while delivering individualized therapy services in patients' homes.

Primary Responsibilities

In this role, you will:

Required Qualifications

Candidates should meet the following requirements:

• Current, unrestricted Physical Therapist Assistant license in the state of Illinois.
• Graduate of an accredited PTA educational program recognized by the American Physical Therapy Association (APTA).
• Minimum of six months of PTA experience in a healthcare setting preferred.
• Strong verbal communication, interpersonal, and clinical documentation skills.
• Reliable transportation with a valid driver's license and current automobile insurance.
• Ability and willingness to travel throughout the assigned coverage area to provide in-home patient care.
