Job summary

One Home Health in Skokie IL is seeking a compassionate Physical Therapist Assistant (PTA) to join our growing home health team serving Skokie and surrounding communities.

You will provide PTA services under supervision of a licensed PT, help patients improve mobility at home, educate families, and document progress. This role supports flexible scheduling with options for full-time, part-time, or PRN.

Qualifications

• Excellent communication with patients and families.
• Ability to document patient progress clearly.
• Will travel to patient homes and communities.

Home Health Physical Therapist Assistant (PTA) | Skokie IL

One Home Health is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our growing home health team serving Skokie, and surrounding communities.

Pay: $45-$55 per hour
Schedule: Full-Time, Part-Time, or PRN

Locations: Some location may include, Skokie, Morton Grove, Des Plaines, Lincolnwood, and surrounding areas.

What You'll Do

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist.

• Help patients improve mobility, strength, balance, and independence in their homes.

• Educate patients and caregivers on exercises, safety, and fall prevention.

• Complete documentation and communicate patient progress with the care team.

Qualifications

• Active Indiana PTA license.

• Current CPR/BLS certification.

• Valid driver's license, reliable transportation, and auto insurance.

• Strong communication and documentation skills.

• Ability to travel throughout Porter County.

Preferred

• Home health experience.

• Knowledge of Medicare documentation requirements.

• Experience with Axxess software.
