Job summary

One Home Health LLC in Chicago seeks a compassionate Home Health PTA to work in Downtown Chicago and North Side neighborhoods, from Lakeview to River North. You will deliver in-home physical therapy, support rehab goals, and collaborate with a multidisciplinary team in a flexible, patient-centered role.

Responsibilities include mobility improvement, patient education, and timely documentation. Requires active IL PTA license, CPR/BLS, and a valid driver’s license with reliable transportation.

Qualifications

• Active Illinois PTA license required.
• Graduation from an accredited PTA program recognized by the APTA.
• Minimum six months PTA experience.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Deliver PTA services under supervision of a licensed Physical Therapist.
• Help patients improve mobility, strength, balance and independence.
• Observe and report progress and changes to the supervising PT.
• Complete clinical documentation in a timely manner.
• Instruct patients on exercises and home programs.
• Collaborate with health care team to coordinate care.

Compensation & Schedule

Pay: $45.00 - $55.00 per hour
Job Type: Part-Time or PRN/Per Diem

About the Role

As a Home Health PTA, you'll work closely with patients in their homes, helping them improve mobility, regain independence, and achieve their rehabilitation goals. You'll be supported by an experienced team of Physical Therapists, nurses, and healthcare professionals who share a commitment to delivering exceptional patient care.

Key Responsibilities

• Deliver physical therapy treatments and patient education under the direction of a licensed Physical Therapist and according to the established plan of care.
• Help patients improve mobility, strength, balance, endurance, and functional independence.
• Observe and report patient progress, responses to treatment, and any changes in condition to the supervising Physical Therapist.
• Complete all clinical documentation accurately and within agency timelines.
• Educate patients and caregivers on therapeutic exercises, fall prevention, safety techniques, and home exercise programs.
• Collaborate with Physical Therapists, nurses, physicians, and other healthcare professionals to coordinate care.
• Participate in care planning discussions and interdisciplinary team meetings as needed.
• Follow all safety, infection prevention, and confidentiality standards.
• Maintain compliance with agency policies and state and federal regulations.

Qualifications

Required

• Active Illinois Physical Therapist Assistant (PTA) license.
• Graduate of an accredited Physical Therapist Assistant program recognized by the APTA.
• Minimum of six (6) months of PTA experience.
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong communication, documentation, and interpersonal skills.
• Ability to travel throughout the Downtown Chicago service area.
• Ability to work independently while providing exceptional patient care.

Preferred

• Home health experience.
• Experience working with geriatric and rehabilitation patients.
• Knowledge of Medicare home health documentation requirements.
• Experience with Axxess software.
• Bilingual candidates are encouraged to apply.
