One Home Health is seeking a compassionate Physical Therapist Assistant (PTA) to join our home health team serving Lake County Illinois and surrounding communities. The PTA will provide treatments under a licensed Physical Therapist, help patients improve mobility and independence at home, and educate patients and caregivers on exercises and safety.

Ideal candidates hold an Indiana PTA license (or eligible), CPR/BLS certification, a valid driver's license, and strong documentation skills.

Qualifikationen

• Valid driver's license, reliable transportation, and auto insurance.
• Strong communication and documentation skills.
• Ability to travel throughout Porter County.

Home Health Physical Therapist Assistant (PTA) | lake County IL

One Home Health is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our growing home health team serving Lake County Illinois, and surrounding communities.

Pay: $45-$55 per hour
Schedule: Full-Time, Part-Time, or PRN

What You'll Do

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist.

• Help patients improve mobility, strength, balance, and independence in their homes.

• Educate patients and caregivers on exercises, safety, and fall prevention.

• Complete documentation and communicate patient progress with the care team.

Qualifications

• Active Indiana PTA license.

• Current CPR/BLS certification.

• Valid driver's license, reliable transportation, and auto insurance.

• Strong communication and documentation skills.

• Ability to travel throughout Porter County.

Preferred

• Home health experience.

• Knowledge of Medicare documentation requirements.

• Experience with Axxess software.
