Job summary

One Home Health in Chicago seeks a compassionate Physical Therapist Assistant (PTA) to join our home health team serving Lakeview, Lincoln Park, Logan Square, Old Town, West Loop, River North and surrounding communities.

Provide physical therapy treatments under the supervision of a licensed Physical Therapist, help patients improve mobility and independence at home, educate on exercises and safety, and complete documentation for the care team. Travel within service areas is expected.

Qualifications

• Active Indiana PTA license.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Ability to travel throughout Porter County.

Responsibilities

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist.
• Help patients improve mobility, strength, balance, and independence in their homes.
• Educate patients and caregivers on exercises, safety, and fall prevention.

Home Health Physical Therapist Assistant (PTA)

One Home Health is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our growing home health team serving Lakeview, Lincoln Park, Logan Square, Old Town, West Loop, River North and surrounding communities.

Pay: $45-$55 per hour
Schedule: Full-Time, Part-Time, or PRN

Location: Areas include Downtown Chicago: Lakeview, Lincoln Park, Logan Square, Old Town, West Loop, River North and surrounding communities.

What You'll Do

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist.

• Help patients improve mobility, strength, balance, and independence in their homes.

• Educate patients and caregivers on exercises, safety, and fall prevention.

• Complete documentation and communicate patient progress with the care team.

Qualifications

• Active Indiana PTA license.

• Valid driver's license, reliable transportation, and auto insurance.

• Strong communication and documentation skills.

• Ability to travel throughout Porter County.

Preferred

• Home health experience.

• Knowledge of Medicare documentation requirements.

• Experience with Axxess software.
