Job summary

One Home Health Agency, Ltd. seeks a compassionate Physical Therapist Assistant (PTA) to join its home health team as a part-time clinician serving patients in the Downtown Chicago area (Northside, Lake View, Lincoln Park, Logan Square, and surrounding areas).

You will deliver physical therapy treatments in patients' homes, document visits, and collaborate with nurses and therapists to ensure coordinated care.

Qualifications

• Active Physical Therapist Assistant license in Illinois.
• Graduation from an APTA-recognized PTA program.

Responsibilities

• Deliver PT treatments and educational support in patients' homes per physician care plan.
• Complete visit documentation accurately and on time.
• Observe and report patient responses and progress to supervising PT.
