Continuum Rehab Group is seeking a Physical Therapist to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities. You will deliver evidence-based care and develop rehab programs to enhance residents’ recovery and quality of life.

Qualifications

• Graduate from an accredited university with a Doctor of Physical Therapy or equivalent.
• Valid license in the appropriate state or in process.
• Open to therapists at all experience levels.

Responsibilities

• Utilize evidence-based practice to guide clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment to patients.
• Deliver skilled therapy services to patients needing rehabilitation.
• Offer consultation and counseling to patients, families, and caregivers.
• Supervise Physical Therapy Assistants and students per licensure requirements.

Experience/Qualifications:

Must have graduated from an accredited University and have valid (or in process) License in appropriate state.

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements.
• Provides skilled therapy services to patients who need rehabilitation services.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
