Job summary

Continuum Rehab Group is seeking a Physical Therapist to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities.

You'll provide evidence-based therapy, supervise assistants, and contribute to rehab programs that improve residents' recovery and quality of life. We offer mentorship, a sign-on bonus, and a comprehensive benefits package including 401k, PTO, medical, dental, and vision coverage.

Must have graduated from an accredited University and have valid (or in process) License in appropriate state.

Experience/Qualifications

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements.
• Provides skilled therapy services to patients who need rehabilitation services.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
