Job summary

Continuum Rehab Group is seeking a Physical Therapist to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities. The role emphasizes compassionate patient care and a collaborative clinical culture in a supportive environment.

Qualifications

Experience/Qualifications:

Must have graduated from an accredited University and have valid (or in process) License in appropriate state.

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.

• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements.

• Provides skilled therapy services to patients who need rehabilitation services.

• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.

• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards
