Continuum Rehab Group is seeking a Physical Therapist to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities. The role supports compassionate patient care and professional growth within a supportive, family-like culture.

Qualifications

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment to patients with physical functioning disorders.
• Provides skilled therapy services to patients who need rehabilitation services.
• Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
• Supervise Physical Therapy Assistants, temporary licensees, students and support personnel per licensure requirements.

Experience/Qualifications:
