Job summary

Continuum Rehab Group in Taos, NM is seeking a Licensed Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities.

You will work under the supervision of a Physical Therapist to provide evidence-based, medically necessary treatment and to support residents' recovery and quality of life. The position welcomes candidates at all experience levels and offers a comprehensive benefits package.

Qualifications

• Licensed Physical Therapy Assistant credential required.
• Open to therapists at all experience levels.
• Experience in skilled nursing/long-term care preferred.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders under supervision of a Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and documents patient progress and communicates with supervising Physical Therapist to adjust treatments.
• Provides consultation to patients and families.
• Input to supervisor for discharge summaries per regulatory requirements.

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
