Qualifications

• Active Connecticut Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical judgment and teamwork skills.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized plans of care.
• Provide skilled interventions focusing on strength, balance, gait, transfers, endurance, and mobility.
• Establish functional goals and monitor resident progress throughout treatment.
• Modify treatment plans based on clinical needs and response to therapy.
• Educate residents and caregivers on exercises, safe mobility, and fall-prevention strategies.
• Complete accurate and timely clinical documentation.
• Collaborate with PTAs, OT, SLP, nursing, and other care team members.
